All Adult memberships require:

  • government picture identification
  • proof of Jasper residency or employment

Parent/guardian must be present when applying for a juvenile card.

Card holders are responsible for ALL items and charges on their card, and for keeping their contact information up-to-date.

√* -  age 10 and under, the parent/guardian must be with the child while they are on the internet, between ages 11 and 14 the child must have their parent/guardian come in and sign a permission form.
